Why is this rule not being enforced?

This is a new rule.

Basically, if you've had other names during your history at the Phora, Lyceum, etc, hell, even back in the days before the Phora, put them all in your sig. Like so:

FORMERLY: DeathKnight1488, Lucius Caecilius IreadhistoricalfictionsetinRomicus, Fred.

Honestly, it will make things a lot easier.

People are required to put their old names in their profiles before applying for a namechange.

Correct, as clicking on Stan's name shows:
The poster formerly known as Constantinus, CONSTANTINUS, CONSTANTINVS MAXIMVS, King Leopold II, Napoleon Bonaparte, Tomasz de Torquemada, Locutus of Borg, Godfrey of Bouillon, St. Pius X, D-FENS, Charles Martel, Stan, Cesare Borgia and of course Bob.

Ideally, former usernames should go in both profile, and signature.

Is it left to the user to do this or does a moderator see that it is done? I occasionally see posters with 2,000 posts and more whose name I don't recognize and whose profile and signatures contain no previous names, which is unlikely.

You're supposed to add them yourself. We check if they're there when we do the namechange. unfortunately, some people remove them again later on. We can't do much else other than denying further namechanges to those people. Removing those old names from your profile is quite rude, and I urge people not to do it.
