I would assume gassing/shooting/violence were the largest cause of death. The other two certainly were responsible for many deaths. The Nazis, of course, were responsible for all deaths.

Holocaust means to die through fire---a bullet, bacterium, and some cyanide gas doesn’t count.
The only Holocaust during WW2 happened over the cities that were either nuked or were pelted with incendiary explosives (such as Dresden).
Now that leads to a new question---were the NAZIS immoral? No. If those Jews lived then others would be prevented from doing so (since, only a finite of resources exist on this planet).
If you kill you save—if you save you kill. There is nothing morally repugnant just the harsh reality that humans compensate by fucking like bunnies to fill in empty spaces.

Well, if this is what Holocaust denial is reduced to, I suppose it will save me time. Since a key part of Nazi barbarism was the burning of corpses, it fits your inaccurately narrow definition.
And no, Holocaust doesn't necessarily mean death by fire. It means:

The definition means to die through fire—is a cremator committing a Holocaust?
The definition was modified to include the Jews—notice how it specifically mentions the Jews and no other group. Also, any dictionary before WW2 would not include that definition (‘mass killing’)—this is a fairly new addition as well to justify their original blunder—stating that Jews were baked alive.

What about the Greek word implies that one need be KILLED by fire, or that anyone needs to be killed at all? The word implies destruction.
If you are referring to the new definition—well, the Greeks would not have intended that word to be used in such a matter.
We're English speakers, not Greeks. The word means what we as English speakers define it as, in the context of our times.
By the way, we have one definition on the table. If that definition is all we're working with, this issue isn't even debatable. I'm right. :p

They did use the word genocide. As to the word Holocaust, I believe it was adopted by the Jews because of THIS definition:
